Gray Matter Volume Loss Is Increased in Younger Patients With Relapsing Multiple Sclerosis but Minimized by Ozanimod: Experience From the Ozanimod Phase 3 Program

Assess effects of ozanimod across age groups on gray matter volume change in patients with relapsing multiple sclerosis (RMS) in RADIANCE, a 24-month, phase 3 study of ozanimod. 
Brain volume (BV) changes in RMS patients differ across age groups. Limited prospective, longitudinal data exist for cortical gray matter (CGM) and thalamic volume (TV) changes. 

In RADIANCE (NCT02047734), a multicenter, randomized, double-blind study, RMS patients aged 18-55y received ozanimod HCl 0.5 or 1 mg/d or intramuscular interferon β-1a 30 µg/wk. Treatment effect on serial BV was evaluated for ozanimod HCl 1 mg versus interferon by patient age (≤25y, >25-<35y, ≥35y) at baseline in this post hoc analysis of 12- and 24-month MRI data.  

There were 1313 patients (16.2% ≤25y; 31.8% >25-<35y; 52.0% ≥35y) in the ITT population (ozanimod HCl 1 mg, n=433; ozanimod HCl 0.5 mg, n=439; interferon, n=441). Patients ≤25y had greater BV, but more active disease (GdT1 lesions) at baseline. Compared with older groups, patients ≤25y experienced greater BV loss at 12 and 24 months; changes in TV varied more by age than did changes in CGM. At 24 months, ozanimod HCl 1 mg reduced BV loss vs interferon across all age groups in all structures evaluated. Compared with interferon, between-group differences in adjusted mean percentage change from baseline in CGM in ozanimod-treated patients in each age category were 0.88 (0.517?1.240), 0.68 (0.399?0.954), and 0.63 (0.434?0.830) at 24 months; for TV loss, corresponding differences were 0.71 (-0.116?1.539), 0.49 (-0.150?1.125), and 0.40 (-0.051?0.858) at 24 months.
RMS patients ≤25 years of age had greater BV changes than older patients, particularly loss of CGM and TV. Ozanimod HCl 1 mg reduced BV loss across all age groups vs interferon; the effect was greatest for CGM and TV in the younger patient population.
