A cross sectional analytical study of cognitive impairment in survivors of spontaneous intracranial hemorrhage (ICH).

Aim : To study the frequency of cognitive impairment in a cohort of patients hospitalized for spontaneous ICH.

 

Objectives : 

To evaluate the domains of  Learning and memory, Language, Executive function,  Complex attention Perceptual-motor Social cognition in survivors of Stroke due to Intracerebral hemorrhage


Spontaneous Intracerebral haemorrhage survivors often are not assessed for cognitive abnormalities which may have an effect on their functional status. So its proposed to do a cross sectional study of ICH survivors after they are discharged from hospital and report for follow up
A cross sectional analytical study was performed in patients with spontaneous ICH and subjected to cognitive testing. Cognitive impairment is defined if evidence of cognitive decline exists from a previous level of performance in one or more cognitive domains: Learning and memory (or) Language (or) Executive function (or) Complex attention (or) Perceptual-motor Social cognition. Addenbrook cognitive examination scale was used to assess cognitive domains. Survivors of spontaneous ICH in the age group of more than 18 years after 6-12 months after date of ictus were included and secondary causes of ICH were excluded
 Total 120 patients were recruited of which 44 (36.66%) were female. Mean age of study population is 62.3yrs (26-86yrs). And it was found that seventy eight percent were subcortical, Fifteen percent were cortical, and six percent were infra tentorial in location. Twenty eight percent were found to have cognitive impairment, of which 58 % cortical, 22%  sub cortical, and 29% are  infra tentorial in location. The most common domains affected are Executive function followed by working memory .
The prevalence of cognitive impairment in general population was found to be 5-7%, in comparison to high prevalence of cognitive impairment following spontaneous ICH which is 28%, and this cognitive impairment needs to be studied further to accurately prognosticate their functional ability
