Retrospective longitudinal assessment of MG-ADL score with treatment of myasthenia gravis

To assess the feasibility of retrospectively extracting the Myasthenia Gravis Activities of Daily Living (MG-ADL) score from the electronic medical record for purposes of monitoring clinical improvement of large populations of myasthenia gravis (MG) patients.
The MG-ADL scale is a validated eight-item questionnaire which assesses the symptoms and activities of patients with MG and correlates well with functional impairment from MG symptoms. A two-point change in the MG-ADL is thought to be clinically meaningful, with higher scores indicating worse function. Patient reported outcome measures such as MG-ADL have been utilized frequently as an outcome measure in MG research. More recently these have been used in routine clinical care.
MG-ADL scores are routinely obtained and inserted in the documentation flowsheet of the electronic medical record at clinic visits for MG patients in the KUMC neuromuscular clinics. At each clinic visit where MG-ADL scores were obtained we abstracted MG-ADL values, demographic information, serology, and interventions received. Descriptive statistics were used to define baseline characteristics and analyze the data. 
A total of 340 patients with MG who had documented MG-ADL scores were included in this study. A total of 335 patients had data at the initial visit, 90 at 3 months, 99 at 6 months, 74 at 9 months, 124 at 12 months, 106 at 18 months, and 17 at 2 years. Median age at first encounter was 67.0 years with interquartile range of 55-75. Thymectomy was performed in 4.7% of patients. Median MG-ADL score at first visit was 4. Median MG-ADL at 3, 6, 9, 12, 18, and 24 months was 5, 4, 4, 4, 4, and 4.
It is feasible to extract MG-ADL scores from our EMR. Median MG-ADL scores suggest that overall, most cases have mild MG that remains stable over more than 1 year.
