Increasing Resident Satisfaction with Continuity Clinic in Adult Neurology and Internal Medicine Residency Programs through Block Scheduling

To determine resident satisfaction with continuity clinic using the new neurology ACGME approved block scheduling design.
There is little data analyzing Resident continuity clinic schedules and their overall satisfaction with their residency program. Recently, the ACGME RRC for neurology expanded schedule flexibility for continuity clinic to include a block curriculum (every 6 weeks with a minimum of 40 per year). This study was aimed at neurology and internal medicine residents at Geisinger Medical Center in Danville, PA.  The residents participated in a block continuity clinic schedule which replaced the previous ½ day per week design. Pre and post surveys were conducted to determine whether or not overall resident satisfaction towards the continuity clinic improved after the curriculum changed. 
Neurology and Internal Medicine residency programs at Geisinger Medical Center in Danville, PA instituted a block curriculum; four to six weeks of inpatient or elective rotations followed by one to two weeks of continuity clinic. This differed from the traditional continuity clinic schedule of one-half day of continuity clinic every week for the whole year. Surveys were conducted both before and after the curriculum change, which looked at resident satisfaction of continuity clinic.  
There were a total of 91 responses (56 pre survey and 35 post survey).  Overall resident satisfaction towards the continuity clinic was improved after block curriculum as compared to the longitudinal curriculum prior (79% vs 38%p 0.0004). Overall dissatisfaction with the time the residents spent with their inpatients on non-clinic weeks decreased significantly (32% vs 14%, p 0.0122). 

Changing the residents schedule from ½ day per week to dedicated block curriculum for continuity clinic improved overall resident satisfaction as well as decreased dissatisfaction among resident’s time with inpatients.  The Neurology RRC decision to allow flexibility in continuity clinic schedules has positively impacted our residents at Geisinger Medical Center.
