A Hospital Based Cross-sectional Study Among Women With Epilepsy In Addis Ababa, Ethiopia

Objective: Describe specificity of epilepsy, quality of care and knowledge among women with epilepsy (WWE) in Ethiopia.

Background: Women with epilepsy face additional burdens. Epilepsy affects sexual development, menstrual cycle, aspects of contraception, fertility and reproduction.  Therefore, the clinical care of WWE entails special considerations over their life span. So far, there is no of data in Ethiopia about these, the quality of care and awareness of these issues among women with epilepsy.
Methods: A descriptive Hospital based cross sectional study was done through interviewing and reviewing clinical data of WWE  at the Neurology Referral Clinic in Black Lion Hospital from February, 2013 till October, 2013 . The KOWE I was used to assess the knowledge of WWE . Data entry and appropriate statistical analysis was performed using the 19.0 version of the SPSS.

Results: 180 WWE, mean age of 29.9 +12.5 were included. 60% were never married and 60.6% were unemployed. In 31.6% seizure onset was 10 to 14 years. 12.4% had Catamenial Seizure. 31.6% had menstrual disturbance. 20.8% had fertility problem, mainly attributed to psychosocial factors. 46.9% used contraception methods where 37.8% used  COCP. 96.6% were never counselled as to choice of contraception. 25.6% had a history of contraception failure.  23.2% were not on follow up during pregnancy. Only 15.38% recall receiving preconception counselling. Baseline serum AEDs level was determined in only one participant. Only 2 received folic acid. During pregnancy 69.4% were on monotherapy, Valporate was avoided in 94.4%, vitamin K was not provided in all .  During labour & delivery 75% continued their AEDs. 83.36% had breastfed. History of bone fracture was in 7.2% , none were supplemented Vitamin D.  The mean knowledge score was 3.4+ 1.9 and 56% had poor knowledge.

Conclusion: Women related issues of epilepsy are under treated, under recognized and  most women are not knowledgeable.
