Clinical Features of Headaches in Elderly Patients. PREMECEF Results.

Identify the differences in clinical characteristics between headaches of previous onset or after 65 years old.
The onset of a new headache in elderly patients is suggestive of a secondary headache, making it necessary to investigate the clinical features to make the correct diagnosis and appropriate treatment. However, there is a lack of information about headache in this age group, because most of the prevalence studies include few or no older adults.
Prospective study of cases due to headache consultation by people > 65 yo at the University Hospital “Dr. José E. González”, diagnosed by the criteria of the ICHD-III, included in PREMECEF (First Mexican Headache Registry), from January 2017 to August 2019. Parameteres studied included headache onset before or after 65 years.
810 patients were registered in PREMECEF, with  57 patients that fulfilled the inclusion criteria. 41(71.9%) were women and average age 74.54±6.11. The most frequent diagnosis was tension-type headache (43.8%), quality of pain most common was oppressive (41.1%), most frequent location was occipital (28%) and 63.1% had at least one episode per day. They were divided into 2 subgroups according to the age of onset, <65 (n = 18) and ≥65 years (n = 39). The most frequent diagnoses were tension-type headache (44% vs. 43.5%) and trigeminal neuralgia (33.3% vs. 10.2%): in terms of secondary headaches it was 5.5% vs. 15.3%. The most relevant clinical features were stabbing pain (33.3% vs. 17.9%), dysautonomic manifestations (16.6% vs. 5.1%) and the identification of some attenuator (5.5% vs. 12.8%).
Trigeminal neuralgia and dysautonomic manifestations were most common in headaches initiated before age 65 and secondary headaches and the presence of pain relievers in the onset group after age 65. The rest of the characteristics are similar in both groups, a greater number of patients is required to elucidate certain differences.
