Title Can Consensus be Obtained on Preferred Practices for Rescue Therapies for Seizures and Seizure Clusters?

Objective -

Background There is no consensus on preferred terms or practices for using rescue therapies (RTs) for seizures and seizure clusters, despite high (but variable) prevalence of seizure clusters and perceived underuse of RTs.

Design/Methods A multidisciplinary group of experts and stakeholders in epilepsy and school health participated in a multistep consensus process led by the Epilepsy Foundation, including multiple workgroups, an in-person workshop, teleconferences and surveys. A modified Delphi technique was used to seek consensus from workgroup participants using an online survey. A statement was considered to reach agreement when 80% or more of respondents rated an item between 6 and 8 on a 0-8 Likert scale.

Results 46 responses were received. Agreement on preferred practices fell into 4 areas: When RTs should be given, importance of a common language, assessing the need for RTs, and education about RTs. Agreement was obtained on items including: “every patient should be assessed for the need for an RT”, and “the use of RTs should be individualized by the provider”. There was also consensus on a revised definition of seizure clusters. The importance of prescribing RTs in specific situations reached agreement for only 4 of 16 items, yet patient specific factors to assess regarding the need for RTs were agreed upon for 10 of 16 items. The majority of respondents felt that all people with epilepsy should be educated about RTs and seizure action plans.

Conclusions An expert consensus process identified agreement on a number of critical aspects for using rescue therapies. A revised definition of seizure clusters has also been identified. These findings are being tested in much larger sample of experts who prescribe rescue therapies and those who educate or administer therapies. Results will be used to publish a consensus statement and to guide proper and safe use of rescue therapies and educational initiatives.
