Enhancing Resident Exposure to Outpatient General Neurology through an Ambulatory Urgent Care Rotation

To broaden resident exposure to general neurology through an innovative outpatient urgent care experience.
There is a trend toward pursuit of sub-specialization fellowship as opposed to general neurology following residency. This may partly be due to lack of exposure to outpatient general neurology in training. We sought to broaden residents’ exposure and further hone their diagnostic skills in an academic ambulatory setting.

NYU Langone Health PGY3 and PGY4 residents completed two weeks of supervised outpatient general neurology for patients with new presentations (less than 3 months of symptoms, not previously seen by a neurologist). Residents were then surveyed to assess the impact of the initiative on general neurology exposure, and preparedness to practice as a generalist. Participants ranked the contribution of the rotation to their outpatient experience on a 5-item Likert scale: “1”-detracted from the experience to “5”-significantly enhanced their experience.


Between September 2017 and August 2018, 506 patients were seen. Frequent diagnoses included concussion, headache syndrome, neuropathy, vertigo and radiculopathy. Twenty-one of thirty-one residents completed a voluntary, anonymous survey assessing the impact of the rotation. Of the 14 residents who had completed the urgent care rotation at time of survey, all responded positively, with nearly 80% ranking it a “5” on a Likert scale of 1-5, indicating significant enhancement of their outpatient experience. Additionally, residents highlighted various strengths of the rotation: having 60 minutes per patient, seeing a variety of attending clinical styles and serving a distinct patient population.

 This study demonstrated the perceived value of an urgent care ambulatory rotation to further enhance resident exposure to general neurology. This curriculum may serve as a pilot for valuable, educational outpatient exposure in a setting distinct from typical resident-run clinics. Future studies may explore the impact of this rotation on pursuit of general neurology careers.
