Clinical Features of LGI-1 Antibodies Positive Encephalitis with Faciobrachial Dystonic Seizures Turning to the Counterpart Side.

OBJECTIVE: To investigate the clinical feature of faciobrachial dystonic seizure (FBDS), which convert to the contralateral side.

BACKGROUND: FBDS were recognized as the epileptic origin. However, ictal scalp EEG is usually normal. The mechanizms of FBDS were not fully investigated.

 

DESIGN/METHODS: A 72-year-old male with right-sided FBDS due to LGI1-Ab encephalopathy. He had involuntary dystonic movement of right hands and mouth for three months before admission. His serum sodium was normal. Correlation between clinical course and findings of MRI and EEG was serially analysed.

RESULTS: On day 1, His MRI showed T1 hyperintensity of right basal ganglia and T2 hyperintensity of left basal ganglia. On day 9, right basal ganglia was enhanced. On day 10, his EEG showed rhythmic slow activities arising from right frontal region. Midazolam and corticosteroid infusion were effective to FBDS. AEDs were continued. LGI-1 Ab were positive. 5 months after onset, left FBDS was appeared. His EEG showed rhythmic fast activities arising from right frontal region.

CONCLUSIONS: The present study showed that the both type to FBDS were associated with the different EEG pattern.
