Patient-reported Experience with Inpatient Teleneurology: A Pilot Qualitative Study

To understand how patients experience virtual inpatient teleneurology consultations and characterize the potential benefits and barriers to implementation.
In neurology, telestroke has been used since the late 1990s to provide expert-level stroke care virtually; however, inpatient teleneurology has not yet been widely integrated. In June 2020, the Department of Neurology at Wake Forest Baptist Medical Center implemented an inpatient teleneurology consult service to provide expert-level neurology consultation virtually to three satellite campuses lacking in-person neurologists. We characterize the patient experience during the pilot phase of implementation (June 1 – September 30).
A qualitative study was conducted using grounded theory and convenience sampling of all patients for whom a teleneurology consult was completed. Assenting patients underwent telephone interview and were asked four questions about their experience. Retrospective chart review was performed to determine consult characteristics and summarized using descriptive statistics.
Of the 53 teleneurology inpatient consults completed, the median and mean patient age was 68; 64% of consults were for possible stroke, and 57% were diagnosed with a stroke or TIA. The teleneurologist provided new diagnostic and/or management recommendations in 70% of consults. Only 8% of patients were transferred for tertiary-level care. Follow-up interviews were performed with 25 patients; 72% endorsed that the virtual neurology consultation improved understanding of their condition and/or treatment. Four patients (16%) stated a preference for the virtual consultation due to the convenience of not being transferred. Only two patients (8%) reported technological barriers.    
This teleneurology consult program was well-received by patients who could be successfully interviewed. Technological concerns were not major barriers, and patients appreciated that hospital transfers were minimized. Based on this initial feedback, the consult service was continued beyond the initial pilot. Further investigation will characterize the impact on transfer volumes and patient outcomes to determine the role of inpatient teleneurology after the pandemic.
