Title Association of the Mediterranean Diet and Cognition Function among Hispanics/Latinos

Objective Investigate the associations between adherence to the Mediterranean Diet (MeDi) with cognitive performance among Hispanics/Latinos.

Background

High adherence to the MeDi has been associated with lower cognitive impairment. However, Hispanics/Latinos have rarely been studied.

Design/Methods We included 6,321 participants aged 45-74 at baseline from the Hispanic Community Health Study/Study of Latinos (HCHS/SOL) ancillary study, the SOL-Investigation of Neurocognitive Aging (SOL-INCA). Participants completed 24-hour dietary recalls at baseline (2008-2011). Food and nutritional components were used to calculate a MeDi score (MeDiS). MeDi adherence was categorized as low (MeDiS=0-4), moderate (MeDiS=5-6) or high (MeDiS=7-9). Neurocognitive tests (Brief-Spanish English Verbal Learning Test Sum [B-SEVLT-Sum], B-SEVLT-Recall, Word Fluency [WF], and Digital Symbol Substitution [DSS]) were administered at baseline and follow-up (2015-2018). Cognitive change was calculated using (cognitive score at follow-up minus predicted cognitive score at follow-up)/root mean square error. Complex survey linear regression analyses were used to determine the association between MeDi adherence groups and z score-transformed cognitive outcomes as well as cognitive change after adjusting by age, sex, education and baseline sociodemographic and clinical characteristics.

Results

Mean baseline age±SE was 56.1±0.2 years and 42% were males. Mean MeDiS±SE was 5.05±0.04. MeDi adherence frequencies were 32%, 45%, and 25% for low, moderate and high, respectively. The average elapsed time between visits was 6 years. Compared to the low adherence group, adults with high adherence had higher B-SEVLT-Sum (β=0.16; p<0.001) and B-SEVLT-Recall (β=0.20; p<0.001) scores at baseline, and higher B-SEVLT-Sum (β=0.21; p<0.001) and B-SEVLT-Recall (β=0.23; p<0.001) scores at follow-up. No significant differences were observed for WF and DSS (p>0.05). Cognitive change in the low vs. high MeDi adherence groups was significant for B-SEVLT-Sum (β=0.13; p<0.01) and B-SEVLT-Recall (β=0.15; p<0.01), but not for WF and DSS (p<0.05).

Conclusions Among diverse middle-age and older Hispanics/Latinos, high MeDi adherence was associated with higher episodic learning and memory and decreased 6-year memory decline.
