A prospective observational study using questionnaires on the treatment needs of patients with dementia with Lewy bodies, their caregivers, and their physician in Japan

We investigated the treatment needs of patients with dementia with Lewy bodies (DLB) and their caregivers and clarified whether physicians understand those.

Physician should focus on various symptoms because the clinical manifestations are diverse on DLB. And there are some problems of pharmacotherapy including severe sensitivity and adverse event. Physician need to understand the treatment needs of patients and caregivers under those environment, however, there have been no reports of directly investigating those.

We selected 52 clinical symptoms and classified them into 7 symptom classification (cognitive impairment, parkinsonism, psychiatric symptoms, eating behavior-related problems, sleep-related disorders, autonomic dysfunction and sensory disorders). The subjects were patients with probable DLB, their caregivers and their physicians. Patients were selected from outpatients visiting 33 facilities to which a DLB specialist belongs. We conducted a questionnaire survey that included a question like “Please select only one symptom that currently causes you the most distress” to patients and caregivers. And physicians were asked for only one symptom that the physician’s patient and caregiver were most distress with. The primary endpoints were the patient-physician and patient-caregiver match rates for the "patient's most inconvenient symptom classification" and the caregiver-physician match rates for the "caregiver's most inconvenient symptom classification".

272 patients and caregivers and 43 physicians participated. The patient-physician and patient-caregiver match rates for the "patient's most inconvenient symptom classification" were 46.9% and 65.7%, respectively. The caregiver-physician match rates for the "caregiver's most inconvenient symptom classification" were 50.8%. The most inconvenient symptom classification of patient and caregiver were parkinsonism and psychiatric symptoms, respectively.

This study is the first report regarding the treatment needs of DLB patients, their caregivers. This result might indicate that it is extremely difficult for physician to understand those, however, this result may greatly contribute to the progress of DLB medical care.
