Middle meningeal artery embolization performed through a transcarotid artery revascularization stent (TCAR)

Our objective is to demonstrate a case in which middle meningeal artery embolization was successfully performed through a transcarotid artery revacularization (TCAR) stent.  

A 76-year-old man presented with left upper and lower extremity weakness. He was found to have infarcts in the right frontal and left parietal lobes. Occlusion of the right internal carotid artery was noted on the CT angiogram. The patient underwent transcarotid artery revascularization (TCAR). Twelve days after the procedure, the patient developed a left hemispheric subdural hematoma.

A middle meningeal artery (MMA) embolization was targeted in order to decrease hematoma expansion. Embolization of the left MMA utilizing PVC particles was performed. A 5F Berenstein diagnostic catheter was advanced through and past the TCAR stent, which overlaid the origin of the left external carotid artery. The frontal, parietal, and petrosquamosal trunks of the MMA were then occluded utilizing PVC particles.

MMA embolization was successfully performed in a situation in which a TCAR stent was found to overlay the origin of the left external carotid artery.

This case report shows that successful MMA embolization is possible through a TCAR stent utilizing a variety of diagnostic catheters without compromising the integrity of the TCAR stent.
