Epidemiological Characteristics of Patients Received Therapeutic Hypothermia Following Acute Ischemic Stroke - a Nationwide Perspective

Aim of our study is to evaluate characteristics of the patients received Therapeutic hypothermia(TH) amongst acute ischaemic stroke(AIS).
Stroke remains the leading cause of morbidity and mortality in the US and worldwide, to date, very few pharmacologic or non-pharmacologic measures have proven neuroprotective in AIS. Therapeutic hypothermia(TH), a cooling technique to reduce body temperature to below 36°c, decreases the oxygen requirement of cells by slowing down their metabolic rate, thus helping reduce cellular apoptosis and inflammation during acute brain ischemia. 
A cross-sectional study was conducted on Nationwide Inpatient Sample(2003-2017). Adults with hospitalizations with primary admission with AIS were extracted and the same hospitalization TH as management was identified using ICD-9 and 10-CM codes. Weighted analysis using chi-square test and unpaired t test was performed to evaluate prevalence and characteristics of the patients received TH amongst AIS. SAS software was used and p value<0.05 was considered significant.
Out of a total 5,690,773 AIS hospitalizations, 620(0.01%) patients received TH. In AIS patients, the TH recipients were younger(mean age:59 vs 70-years), TH was more prevalent in age group 18-34(7.5 vs 1.1%), 35-49(20.2 vs 6.8%), male(55.7 vs 47.7%), African American(19.5 vs 17%), Hispanic(12.2 vs 7.7%), Asian or Pacific Islander(8.2 vs 2.7%), and private insurance(30.6 vs 18.9%) in comparison with non-TH.(p<0.001) TH as a management was more frequent in large hospitals(78.5 vs 60.4%), Urban Teaching(85.6 vs 51.4%), and Northeast(26.1 vs 20.4%) and West(33.3 vs 18.6%) region.(p<0.001) Amongst patients, received mechanical thrombectomy(21.2 vs 1.1%) and tPA(16.8 vs 5.6%) the prevalence of TH was higher in comparison without TH.(p<0.001). The TH prevalence was higher amongst smokers(21 vs 16%), alcoholics(4.7 vs 4%), and drug abusers(6.6 vs 2.5%).(p<0.001)
TH is a novel treatment and very little information is known, more prospective studies should be planned to evaluate outcomes and population who can get benefit following TH amongst AIS hospitalizations.
