Decrease in Body Weight With Once-Daily Atogepant for the Preventive Treatment of Migraine: A Post Hoc Analysis

To evaluate body weight following once-daily atogepant for the preventive treatment of episodic migraine.
Atogepant is a novel, oral, calcitonin gene?related peptide receptor antagonist with demonstrated efficacy in the preventive treatment of migraine. Obesity is a common comorbidity in people with migraine and a risk factor for disease progression, including the development of chronic migraine.
ADVANCE was a double-blind, placebo-controlled trial where participants with episodic migraine were randomized to 12 weeks of treatment with placebo or once-daily atogepant 10mg, 30mg, or 60mg. In a 52-week open-label trial, participants with episodic migraine were randomized to standard care (SC) or once-daily atogepant 60mg. In this post hoc analysis, outcomes were least squared (LS) mean percentage change in body weight from baseline to end of treatment and the proportion of participants with a ≥7% change in body weight at any timepoint postbaseline (potentially clinically significant change).
In the ADVANCE trial, LS mean percentage change in body weight after 12 weeks was +0.37 with placebo versus +0.14 (P=0.4138), -0.61 (P=0.0005), and -1.27 (P<0.0001) with atogepant 10mg, 30mg, and 60mg. In ADVANCE, 3.2% of placebo participants had a ≥7% weight loss versus 4.1%, 2.7%, and 5.7% for atogepant 10mg, 30mg, and 60mg. A ≥7% weight gain was observed in 2.7% of placebo participants versus 2.3%, 1.8%, and 0.0% for atogepant 10mg, 30mg and 60mg. In the 52-week trial, LS mean percentage change in body weight after 52 weeks was +0.19 with SC versus -1.57 with atogepant 60mg (P=0.0003). A ≥7% weight loss was observed in 14.7% of SC participants and 24.1% of atogepant 60mg. A ≥7% weight gain was observed in 12.6% of SC participants and 7.3% of atogepant 60mg.
Once-daily atogepant for the preventive treatment of migraine was associated with a dose-dependent decrease in body weight compared with placebo and standard care.
