Title Impact of Celecoxib Oral Solution on Symptomatic Improvements in the Acute Treatment of Migraine

Objective This pooled analysis of 2 multicenter, randomized, double-blind, placebo-controlled clinical trials (ClinicalTrials.gov Identifier: NCT03006276 and NCT03009019) assessed the efficacy of celecoxib oral solution for the acute treatment of migraine on secondary outcomes.

Background Celecoxib is a nonsteroidal anti-inflammatory drug (NSAID) that works via selective cyclo-oxygenase (COX)-2 inhibition. NSAIDs are standard-of-care migraine treatments, but many are associated with adverse gastrointestinal effects and slow absorption. Celecoxib oral solution is a well-tolerated and rapidly available (T_max=0.7 hours) formulation, which may improve gastrointestinal safety and tolerability and provide rapid effect onset.

Design/Methods

Participants reporting 2-8 attacks per month of moderate-to-severe pain intensity were enrolled in each trial and administered celecoxib oral solution (120mg) or placebo within 1 hour of migraine onset. Compared with placebo, celecoxib oral solution demonstrated significant primary efficacy outcomes of freedom from headache pain and most bothersome symptom (MBS) at 2 hours postdose, and no safety concerns were identified in either study. Secondary efficacy outcomes assessed at 2 hours postdose included headache-pain relief and freedom from predose allodynia, nausea, phonophobia, or photophobia.

Secondary efficacy outcomes from the pooled intent-to-treat full-analysis set population (n=1130) were tested for statistical significance using Fisher’s exact test. Odds ratios (ORs) with 95% confidence intervals (CIs) were also calculated.

Results Key secondary efficacy outcomes achieving statistical significance included headache-pain relief (71.3% celecoxib vs 58.1% placebo;p<0.0001; OR:1.80;95%CI:1.40-2.31), freedom from predose phonophobia (52.2% celecoxib vs 36.4% placebo; p=0.027;OR:1.91;95%CI:1.10-3.32), and freedom from photophobia (58.1% celecoxib vs 43.4% placebo; p<0.0001;OR:1.81;95%CI:1.41-2.34). Whereas freedom from predose allodynia and nausea did not achieve significance, additional secondary outcomes did, including absence of predose MBS, phonophobia, or photophobia; absence of screening MBS or photophobia; and headache pain relief.

Conclusions Pooled analyses of 2 randomized, double-blind, placebo-controlled trials demonstrated extensive efficacy of celecoxib oral solution for acute treatment of migraine and support the co-primary efficacy findings.
