Impact of pre-existing small vessel disease on Motor Outcomes after Deep Brain Stimulation in Parkinson’s Disease

To assess the impact of pre-existing small vessel disease on motor outcomes after deep brain stimulation in Parkinson's Disease 
Chronic small vessel disease (SVD) is associated with poor motor function in Parkinson’s Disease (PD). Deep Brain Stimulation (DBS) is  effective  in improving motor symptoms and quality of life. However, the impact of SVD on PD patients undergoing DBS is unclear. We sought to evaluate if the severity of small vessel disease had an impact on motor outcomes in PD patients undergoing DBS. 
We retrospectively reviewed consecutive patient data for patients with PD who underwent DBS surgery in a single center cohort. Demographic data as well as cerebrovascular risk factors (hypertension, diabetes, atrial fibrillation), medications(Anticholinergic agents, amantadine, carbidopa/levodopa, COMT inhibitors, dopamine agonists, MOA inhibitors),Hoehn and Yahr scale and  Unified Parkinson’s Disease Rating Scale (UPDRS) Motor Score with and without medication before and after DBS were collected. Multiple linear regression (MLR) was used to determine factors associated with post DBS UPDRS motor score.
A total of 90 patients were included in the study. Simple linear regression was used to determine variables of interest to include in MLR. Variables significant at the 0.1 level were included in the MLR: age, diabetes, and Hoehn and Yahr stage. Fazekas overall score (mild 0-2 vs. moderate to severe 3-6) was included in the regression model as a variable of interest. Accounting for the effects of age, diabetes, and Parkinson’s stage at time of DBS, Fazekas score was not a predictor of post DBS UPDRS motor score. 
The severity of small vessel disease (SVD) did not impact the UPDRS motor outcomes in Parkinson patients after DBS. Further studies are needed to validate our findings.
