Development of Validated Clinical Outcome Assessments in Limb Girdle Muscular Dystrophy R1/2A

To determine the test-retest reliability of functional motor clinical outcome assessments (COAs) in limb girdle muscular dystrophy R1 (LGMDR1).
LGMDR1 is an autosomal recessive form of LGMD due to mutations in calpain-3 (CAPN3),  resulting in progressive muscle weakness.  Advances in gene replacement technologies raise the possibility of disease altering treatments requiring future clinical trials. A barrier to clinical trials is an incomplete understanding of the natural history of LGMDR1 as measured by functional motor COAs.
Prospective observational study of clinically affected, genetically defined LGMDR1 participants seen at 7 academic centers and followed for up to 24 months.  COAs include the 100-meter timed test, the North Star Assessment for Limb Girdle-type Muscular Dystrophies (NSAD), 4-stair climb, Timed Up and Go,  the Performance of Upper Limb 2.0 (PUL), and handheld dynamometry (HHD, 6 bilateral upper extremity muscles).  The baseline visit was split over 2 days for reliability testing. Participants were classified as ambulators (A) or  difficulty with ambulation (DA) based on baseline 10-meter walk/run < or ≥ 12 seconds. Cross sectional associations were evaluated using Pearson correlation coefficients; Test – retest reliability was evaluated using the intraclass-correlation coefficient (ICC).
Twenty-two (75 target) participants have enrolled. Most are women (68%).  The mean age 37.1 years, mean symptom onset at 12.5 years, and mean diagnostic delay of 6.8 years. Both the NSAD and the PUL had excellent test-retest reliability, with ICCs 0.998 (95% confidence limit [CL] 0.994, 0.999) and 0.972 (95% CL 0.933, 0.988) respectively.  HHD had good to excellent test-retest reliability with ICC range (0.89-0.99). DA performance was lower/weaker on all COAs.
We obtained excellent test-retest reliability for all COAs in this multisite trial for LGMDR1.  Our understanding of stability and potential sources of variability in COA performance across baseline visits will inform future clinical trial design in LGMDR1.
