Dual energy X-ray absorptiometry to measure lean muscle mass in FSHD

1. Determine the association between percent lean muscle mass in the upper/lower extremities and corresponding clinical outcome assessments. 2. Determine the longitudinal change in whole body and regional lean muscle mass over 1 year. 

Facioscapulohumeral muscular dystrophy (FSHD) is a slowly progressive disease of skeletal muscle. Dual energy X-ray absorptiometry (DEXA) can measure lean muscle mass and has been used in prior clinical trials in neuromuscular diseases.

The Readiness to Solve barriers to drug development in FSHD (ReSolve) study is a prospective, longitudinal, observational multisite study (NCT03458832). DEXA scans and functional outcome measurements (six-minute-walk test, time-up-and-go, quantitative muscle strength testing) were available in 160 patients with FSHD at baseline and 80 patients at 12 months for this analysis. 

The median change in percent whole body lean muscle mass was 0.8% with interquartile range [IQR] 0 to 1.6% over 1 year. There was moderate correlation between percent lower extremity lean muscle mass and clinical outcome assessment such as the six-minute-walk test at baseline (r = 0.34).  

DEXA scan is a widely available, cost-effective and sensitive measure of whole body and regional lean body mass. Changes in DEXA scan lean body mass may be useful as a biomarker in future clinical trials in FSHD.
