Title Mobile Integrated Cognitive Kit (MICK): A Digitized Visual Rapid Automatized Naming (RAN) Task for Concussion on the Sidelines

Objective Prior studies have demonstrated reliability of the MICK app, a novel delivery apparatus for two well-characterized rapid automatized naming (RAN) tasks, the MULES (Mobile Universal Lexicon Evaluation System) and the SUN (Staggered Uneven Number test). Here, we present data for MICK app implementation on the sideline in two athletic cohorts.

Background Neurologic disorders such as concussion may require rapid and, in some instances, digitally-administered, testing. Since visual symptoms are common following concussion, sensitive, specific and widely available visual performance measures are needed.

Design/Methods The MICK App is a digitized version of the SUN and MULES tests that is accessible for research on tablet devices. Participants from two cohorts were included: 12 Yeshiva University athletes (intercollegiate athletic competition) and 11 players from the National Women’s Hockey League. The 23 participants completed the SUN and MULES tests on both paper-pencil and on the MICK app via tablet. A random number generator assigned test order for app vs. paper-pencil versions.

Results Average paper-pencil MULES time for Yeshiva athletes was 36.92±6.14s and was 32.82±16.47s for hockey players. Average MULES time on the MICK app for Yeshiva athletes was 33.24±4.54s and was 34.09±3.89s for hockey players. Average paper-pencil SUN time for Yeshiva Athletes was 50.19±9.52s and was 47.16±2.66s for hockey players. Average SUN time on the MICK app for Yeshiva athletes was 49.39±9.24s and was 49.50±10.44s for hockey players.

Conclusions This study demonstrates feasibility for paper-pencil vs. MICK app versions of RAN testing in intercollegiate and professional athletes. Further collaboration with investigators at institutions with Division I athletic programs is underway and will yield larger cohorts with pre- and post-season data. Scores obtained on the sidelines after suspected concussion will be important for MICK app validation so that this may be made widely accessible.
