Online CME-certified Case Challenges in Migraine Increase Knowledge and Competence Among Neurologists and PCPs

We investigated whether an online case-based medical educational activity could improve the knowledge and competence of PCPs and neurologists in the diagnosis and management of migraine.
Migraine is a severe, disabling neurologic disorder that significantly affects health-related quality of life. Primary care and neurology clinicians often manage patients with migraine and previous data indicate that providers in both settings could benefit from additional migraine education, particularly relating to preventive migraine therapies.

We created two patient case scenarios in an online, interactive, text-based CME activity. Using a “test and teach” methodology, clinicians completed multiple-choice questions to test their application of evidence-based recommendations. Detailed, referenced, feedback followed each response. 好色先生al effect was evaluated through a repeated pairs pre- vs post-assessment of responses linked to individual learners, allowing each learner to act as his/her own control. A paired-samples t-test assessed statistical significance to the paired pre-vs post education responses. Cohen’s d was used to calculate the effect size of the intervention. Data was collected from May 5, 2021 - July 13, 2021. 
Participation in the education resulted in a moderate educational effect for neurologists (n=402; d =.62; P <.001) and a large educational effect for PCPs (n=586; d=.87; P <.001). Significant increases in knowledge and competence for both clinician groups occurred in the following areas (P <.05 for all comparisons): identification of migraine characteristics; identifying candidates for preventive therapy based on American Headache Society recommendations; the personalized selection of an anti-CGRP monoclonal antibody for migraine prevention.  23% of neurologists and 35% of PCPs reported improved confidence regarding the ability to personalize the selection of a preventive migraine therapy

A targeted, online, interactive, case-based educational intervention improved the ability of PCPs and neurologists to identify and effectively manage patients with migraine. Future education should continue to address clinical decision-making in the management of migraine.
