Driving Shared Decision-Making and Best Practices for Insomnia: The Impact of Tethered, Aligned 好色先生 on Clinicians, Patients, and Caregivers

To assess the influence of aligned clinician (HCP) and patient-caregiver (P/C) education in addressing the negative impacts of insomnia on patients and their co-habitants through shared decision-making and individualized treatment decisions.

Despite the heavy burden of insomnia on patients’ ability to function the next day and co-habitants’ quality of sleep, it remains largely untreated, especially in primary care practices.

One 60-min HCP activity and four 15-min modules for P/Cs were launched live-online on 11/24 and 12/8/20, respectively, and will remain on-demand for 1 year. Test questions were administered pre-, immediate post-, and 2 mos post-activity. Responses from learner polling and follow-up surveys were analyzed for engagement, lessons learned, and continuing gaps. McNemar tests compared paired responses (pre/post & pre/2 mos) with Cohen’s d for effect size.
As of 9/23/21, 1394 HCPs and 701 P/Cs had engaged with the activities. Six out of 9 knowledge and competence questions showed significant pre to post improvements (34% lift HCPs and 20% lift P/Cs). When asked about behavioral impacts, ~84% of HCPs reported benefits on clinical practice (n=672), with write-in examples related to ability to proactively inquire about sleep during visits, educate on sleep hygiene, and prescribe appropriate treatments. ~57% of P/Cs reported positive impacts on control over decision-making, and 38% reported a positive impact on communication. Examples of improved communication and self-care included keeping a sleep journal, awareness of steps to improve sleep, and improved ability to discuss sleep.  
Post-tests and participant surveys support the positive impact of live-online HCP and P/C education to facilitate SDM related to sleep management. Ongoing education should continue to emphasize the importance of sleep hygiene for optimal patient outcomes and share strategies to individualize insomnia treatment based on comorbidities.
