Resident Driven Quality Improvement: A Self-Sustaining Model for New Milestones

To develop a model for resident-driven quality improvement (QI) in order to meet the new neurology residency milestones put forth by the American College of Graduate Medical 好色先生 (ACGME).

The ACGME updated milestones for graduate neurology medical education in December 2020, with implementation July 1, 2021. One of the most significant shifts in the recent revision was a shift to include more emphasis on systems-based practice (SBP), including QI.

Residents were directed to select a group QI topic. Residents then participated in an interactive QI workshop led by a current PGY-4 to learn the FOCUS-PDCA (Find-Organize-Clarify-Understand-Select/Plan-Do-Check-Act) framework of QI. From this, PGY-3 residents devised and submitted a formal QI proposal and were directed to continue PDCA cycles for this project throughout the year under the direction of the PGY-4 class and with assistance from the PGY-2 and PGY-1 classes. Final results of the project will be analyzed and presented to the neurology department by the new PGY-4 class at the start of the next academic year to teach new residents the FOCUS-PDCA framework, following which residents will select a new QI subject for the year. 
The residents selected the initial management of status epilepticus (SE) for their subject. At the QI workshop, they identified two initial QI objectives: conduct needs assessment for process variability in SE management and increase use of institutional order-set to improve efficiency. They then submitted a formal QI proposal. Chart review and data analysis of interventions remained in process at the time of abstract submission.
This self-sustaining QI model is an effective method for neurology residents to acquire the skills needed to identify, develop, implement, and analyze a QI project. Residents participating in this model should meet at least a milestone level 4 for the QI sub-competency of SBP during PGY4 year.
