Neurocognitive And Psycho-emotional Disorders In Patients With Chiari Malformation Type I

To assess cognitive impairments and psycho-emotional status in adult patients with Chiari malformation type I before surgery.

Cognitive  impairments and affective disorders are often detected in patients with Chiari malformation type I (CM I) in addition to symptoms specific for this pathology when conducting clinical trials.

We examined 24 patients with CM I (14 women and 10 men) and 20 healthy patients (10 women and 10 men) being at the age from 19 to 65 years old. Neuropsychological testing was carried out using the following scales: Hospital Anxiety and Depression Scale, Repeatable Battery for the Assessment of Neuropsychological Status, Montreal Cognitive Assessment.

Gender, age and education were not statistically significant in control group of healthy patients and those having CM I.

Statistically significant changes were recorded in patients with CM type I and healthy individuals when performing neurocognitive and psycho-emotional testing. The scores for measuring learning abilities, memory, speech fluency and pattern copying were highly worse in patients with CM I. Moderate anxiety and mild depression were detected in the psycho-emotional status of patients with CM type I.

Patients with Chiari malformation type I had some difficulties in learning and semantic fluency as compared to healthy patients. The psycho-emotional status was characterized as moderate anxiety with patients showing signs of clinical depression.

It is to be determined how cognitive and psycho-emotional disorders are formed. However, clinical observations and new researches in neuroanatomy provide a key to establishing multiple connections of the cerebellum with various parts of the brain, making it possible to develop rehabilitation and preventive programs for patients with the given pathology.
