Factors Associated with Anxiety after Hemorrhagic Stroke

Identify factors associated with anxiety after hemorrhagic stroke.

Post-stroke anxiety can last months to years and negatively impact quality of life. Awareness of risk factors may improve diagnosis and treatment.

Patients admitted between January 2015 and February 2021 with non-traumatic hemorrhagic stroke (intracerebral (ICH) or subarachnoid (SAH) hemorrhage) were identified and assessed telephonically using the Quality of Life in Neurological Disorders anxiety inventory at 3-months and 12-months post-stroke. We evaluated the relationship between post-stroke anxiety (T-score >50) and preclinical social and neuropsychiatric history, systemic and neurological illness severity, and in-hospital complications.

Of 71 patients who completed the 3-month assessment, 28 (39%) had anxiety. There was a significant relationship between anxiety at 3-months and Glasgow Coma Scale score on admission (14 (IQR 12-15) for patients with anxiety vs. 15 (IQR 14-15) for patients without anxiety (p=0.034), as well as bleed type (23/46 (50%) patients with ICH had anxiety vs. 5/25 (20%) with SAH, p=0.021). Amongst ICH cases, anxiety was associated with larger ICH volume and higher ICH score (25cc (IQR 6-49) vs. 6cc (IQR 3-12), p=0.021, and 2 (IQR 0-3) vs. 1 (IQR 0-1), p=0.037). On multivariable analysis with GCS score, bleed type and neuropsychiatric history, only bleed type remained significant (OR 3.77 (95% CI 1.19-12.05), p=0.024). Of 32 patients who completed the 12-month assessment, 10 (31%) had anxiety. There was fair agreement (κ=0.31) between anxiety at 3-months and 12-months. There was no significant relationship between anxiety at either timepoint and sex, age, race, ethnicity, education, marital status, alcohol or tobacco use, neuropsychiatric history, premorbid mRS score, admission APACHE II score, ICH side, hospital length-of-stay, ICU length-of-stay, or number of days on a ventilator.

Bleed characteristics and neurological examination on admission are associated with development of post-stroke anxiety.
