Investigating a New Risk Factor of Subarachnoid Hemorrhage: Opium Use Disorder

We evaluated the association between OUD and SAH with consideration of other risk factors and subgroups of SAH: aneurysmal (aSAH) and non-aneurysmal (naSAH).

Subarachnoid hemorrhage (SAH) is associated with many known risk factors, including hypertension (HTN) and tobacco smoking. There is a paucity of research about the possible association between opioid use disorder (OUD) and SAH.

In this case-control study, 417 patients diagnosed with SAH and 405 controls without history of SAH or cardiovascular disease were compared. The case and control groups were compared with regard to demographics and risk factors including gender, age, OUD,  HTN, smoking, diabetes mellitus, and hyperlipidemia. Univariate analyses were used to determine the crude association between SAH and individual risk factors. Binary (case vs control) and multinomial (SAH subgroups vs control) logistic regression were utilized to evaluate the adjusted odds ratio (OR) of each predictor.

Univariate analysis showed a significant association between SAH and OUD, HTN, smoking and age younger than 65 years (all p<0.001). Univariate subgroup analysis did not remain significant for smoking and age younger than 65 years in naSAH patients. After adjusting the predictors, a significant association was found for OUD (OR=6.72), HTN (OR=1.95), TS (OR=1.64) and age younger than 65 years (OR=2.23) between SAH patients and controls (all p<0.05). Multinomial logistic regression analysis showed that OUD was more closely associated with aSAH (OR=8.55, p<0.001) than naSAH (OR=3.74, p=0.002).

Our study showed that OUD is a strong risk factor for SA, and patients with OUD were 6.7 times more likely to have SAH. Although OUD is a risk factor for both SAH subgroups, the association is higher for aneurysmal SAH.
