Correlation analysis between multi-dimensional scale for PDD and ADHD (MSPA) and WAIS-III in adult epilepsy

We investigated correlation between degrees of neurodevelopmental impairment and cognitive function characteristics in adult epilepsy patients with difficulty in social life.

Patients with epilepsy are frequently confronted with difficulty in accommodating themselves to various aspects of social life. The multi-dimensional scale for pervasive developmental disorder (PDD) and attention deficit/ hyperactivity disorder (ADHD) (MSPA) is a novel scale to assess individual behavioral characteristics and necessity of support. The MSPA consists of 14 domains; 5 PDD features (communication, sociality, emotion, restricted interests/behaviors, stereotyped/repetitive motion), 2 developmental coordination disorder features (gross motor, fine motor), 3 ADHD features (hyperactivity, inattention, impulsivity), and 4 other features (sensory, sleep cycle, learning, and language development).

Seven patients with epilepsy (4 males, age 34.6 ± 9.8 years (mean ± SD), duration of epilepsy 20.3 ± 8.4 years), who have trouble in taking part in social activities, were investigated. All patients finished their compulsory education for 9 years, while no social problems were pointed out. MSPA features were assessed by a licensed evaluator. Correlation between MSPA domains and WAIS-III variables were analyzed. Pearson’s correlation or Spearman’s rank correlation was used according to normality of variables.

Strong correlation was shown between learning and VIQ (R = −0.78), learning and VIQ−PIQ (R = −0.73), sleep cycle and VIQ−PIQ (R = −0.72), sleep cycle and difference between subcategories (digit span−letter-number sequencing) (R = –0.73), inattention and VIQ−PIQ (ρ = −0.80), and inattention and difference among four index scores (ρ = −0.78). Communication and sociality domains intermediately correlated with difference among four index scores (ρ = −0.63 and ρ = −0.44, respectively).
Patients with epilepsy, even those with controlled seizures, need special support to adopt social life. Index scores and subcategories of WAIS-III can be useful in assessment and care of epilepsy patients to improve social quality.
