Ensuring continuous neurocritical care education and optimal procedural performance for Advanced Practice Providers (APPs) is vital for delivering safe and effective patient care. Here we propose a tiered compensation approach for educational and procedural competencies for APPs working in neurocritical care units. 

Neurocritical care is evolving rapidly. APPs play a vital role in neurocritical care units. APPs come from various training backgrounds and work experiences. Ensuring continuous neurocritical care education and optimal procedural performance for APPs is vital for delivering safe and effective patient care. To date, there are no published data to guide the initiation of such programs. Here we propose a tiered compensation approach for educational and procedural competencies for APPs working in neurocritical care units. Neurocritical care society (NCS) offers excellent educational materials for medical professionals including PONS, NCS podcasts, and NCS On-Call. Structured annual neurocritical care-related educational activities utilizing NCS resources along with documented procedural competence help APPs in their career advancement and to deliver excellent evidence-based care.

Tier 1 (after completion of neurocritical care APP orientation) include completion of Emergency Neurological Life Support (ENLS), Advanced Cardiac Life Support (ACLS), National Institute of Health Stroke Scale (NIHSS), modified Rankin Scale (mRS) certifications renewed every 2 years. Tier 2 includes Tier 1 certifications along with procedural competencies (10 central lines, 10 arterial lines, 4 endotracheal intubations, and 5 lumbar punctures without any peri-procedural complications). Tier 3 constitutes a minimum of 2 years of neurocritical care APP experience excluding the orientation period, annual procedural competencies listed in tier 2 along with completion of annual educational competencies from NCS including 5 PONS credits, 5 NCS podcasts, and 5 Neurocritical Care On-Call chapters.

By incentivizing maintenance of certifications, continuous annual neurocritical care educational and procedural competencies through financial compensation, APP job satisfaction and work force retention can be improved.
