Incidence of Zoster-Associated Limb Paresis (ZALP): A Population-Based Analysis from 2008-2016

To use population-based tools to determine the epidemiological characteristics of zoster-associated limb paresis (ZALP).
ZALP is a recognized complication of herpes zoster (HZ), with considerable associated morbidity. The incidence of ZALP is unknown.
Utilizing a diagnostic code search of the Rochester Epidemiology Project (REP) database, records for all patients in Olmsted County, MN, USA diagnosed with HZ between 2006-2018 were identified. ZALP cases were adjudicated based on individual chart review. Descriptive and summary statics were applied to the epidemiologic data. Incidence rates were age-adjusted and applied to 2010 census data, with projected growth for subsequent years in the study period.
A total of 7136 HZ patients were identified, 27 (0.4%) of whom developed ZALP (11 upper limb, 16 lower limb). Adjusted total ZALP incidence was 1.37 cases/100,000 persons/year, increasing with age to 11.16 cases/100,000 persons/year in patients 80 years of age and older. Overall incidence was unchanged over the study period. ZALP was more common in men (17/27, p=0.007, OR 3.03 [1.36-6.73]). A majority (78%) of ZALP patients developed post-herpetic neuralgia (PHN).
ZALP is an uncommon complication of HZ which, like HZ, has a much higher incidence in patients over 80 years of age. Unlike HZ, ZALP is more common in men. ZALP patients are significantly more likely to develop PHN than HZ patients.
