A Rare Case Report of Miller Fisher Syndrome Overlapping with Myasthenia Gravis

Miller-Fisher Syndrome (MFS), an autoimmune neurological disorder, is a rare variant of Guillain-Barre syndrome (GBS). Miller-Fisher Syndrome has been reported to affect 1 in every 1,000,000 healthy people, as well as 6.6% of all GBS patients. The concurrent development of both myasthenia gravis (MG) and Miller-Fisher syndrome is even rarer, which makes early diagnosis of the overlapping of these two neurological disorders in a single patient challenging. Early diagnosis with the appropriate treatment can result in a significant improvement in the patient's condition. We present the rare case of a 28-year-old male who has developed concurrent MFS and MG. His MFS was diagnosed before his MG due to the exhibition of the notable clinical triad of ophthalmoplegia, ataxia, and areflexia, as well as testing positive for the Anti-GQ1B antibody. He had muscle weakness, fatigue, and tingling and burning in his extremities. Diagnosis of MG was established due to persistent symptoms consisting of fluctuating muscle weakness with repeated use of his extremities, speech that would improve with a short period of rest, a positive result from a single fiber EMG (SFEMG) suggesting a neuromuscular junction disorder, and the exhibition of a significant improvement of his muscle weakness in response to the use of Mestinon even though he has seronegative antibodies including AchR-Abs, Anti-MuSK Ab. Throughout his disease, he was treated with IVIG, Cellcept, Mestinon, and Steroids. He is now fully recovered and has returned to working full time, with only a maintenance dose of 60 mg of Mestinon 4 times a day. We presented this rare case of 2 concurrent autoimmune neurological disorders to help readers avoid delays in the diagnosis and treatment of patients with similar conditions.
