Unlocking Neurology 好色先生 Through Virtual Neurology Escape Room: A Conference Experience

To present a unique, interactive virtual healthcare escape room that combines education and entertainment, making neurology learning engaging, immersive, and fun at the AAN 2025 Neurology 好色先生 Room.

Escape rooms are increasingly used in education to boost trainee engagement through active, knowledge-based learning. They address multiple ACGME core competencies—including medical knowledge, practice-based learning, communication, systems-based practice, and professionalism. A virtual format adds adaptability, fosters collaboration with minimal resources, and is cost-efficient.

The virtual escape room was conducted with team of five participants across training levels, from medical students to PGY-3 residents, all naïve to escape room–style learning. Participants collaborated to solve a stroke alert case by navigating virtual “rooms” representing phases of stroke evaluation. Clinical clues integrated core competencies including NIHSS assessment, imaging review (CT/MRI), thrombolytic therapy indications and contraindications, and foundational concepts in neuropathology, infectious disease, and genetics. Interactive elements, such as crosswords, analytical games, and eponym challenges reinforced learning. The case was designed using Kern’s six-step curriculum and Bloom’s taxonomy to guide a stepwise approach.

Team had 20 minutes to solve the case and generate differential diagnoses, which led them through different clinical pathways. Post case debriefing emphasized stepwise clinical reasoning, localization, and differential diagnosis. A subsequent 20-minute workshop on designing escape rooms was also conducted and received enthusiastic audience engagement and applause.

This project demonstrates how virtual escape rooms can transform neurology education by blending clinical realism with gamification. Participants reported enhanced learning, and the format has since been adopted at multiple institutions. We hope to inspire wider use of ‘edutainment’ to combine education, engagement, and wellness in medical training.
