Practice, Policy, and Ethics
Participants will learn about the American College of Physicians High Value Care Coordination (ACP HVCC) framework. The ACP HVCC aims to improve the experience with referrals to and ongoing care of specialists with the patient-centered medical home model. Participants will learn about shared expectations, pertinent datasets, referral types, specialist roles, and first-call concepts. Examples of implementation will be discussed with participants invited to share their experiences.
